Transperineal laser ablation (TPLA) of the prostate
Ultrasound-guided transperineal interstitial laser ablation, called TPLA, is an innovative method that allows you to successfully treat the symptoms related to benign hyperplasia of the prostate. It consists in the destruction of prostate tissue with coagulative necrosis induced by illumination with laser light emission.
The TPLA uses a low-power laser and a small needle, inserted percutaneously transperineally – through the skin of the perineum, between the base of the testicles and the anus -, therefore it allows to preserve the urethra, unlike what happens with more traditional techniques which employ a transurethral approach.
The surgery is performed under local anesthesia and is almost painless.
With the use of this procedure, the typical side effects, such as, for example, urinary incontinence and bleeding are eliminated. This is why it is a minimally invasive procedure particularly suitable for those who cannot suspend anticoagulant therapy in anticipation of surgery.
Percutaneous photothermoablative treatment also significantly lowers the risk of complications related to the transurethral approach, including urethral stricture, and does not preclude any subsequent treatments.
Compared to standard techniques, greater safety is guaranteed thanks to the minimally invasive procedure. The risks of complications are significantly reduced and a sexual life preserved in the patient suffering from this pathology, which does not only affect the elderly.
TPLA ablation has not only proved to be a valid ally to treat the characteristic and insidious symptoms of BPH early, safely and painlessly, but also allows you to postpone more invasive surgical interventions, which can instead be scheduled at an older age.
