Curved penis​

The curved penis can be present during development (congenital) or acquired through the development of a penile plaque (Peyronie’s disease). The progression of the disease can lead to erectile dysfunction and the impossibility of intercourse. Therapy with clostridium histolyticum collagenase allows it to be cured in 80% of cases. Only in severe cases does patch surgery allow for recovery of the length and natural silhouette of the penis.

Diagnosis

The diagnosis of curved penis is carried out through the vision of erect photos and the dynamic penile Doppler ecocolor for the evaluation of the composition and extension of the penile plaque.

    In November 2016, the first non-surgical treatment based on collagenase was introduced in Florence, an innovative substance that degrades plaque into its collagen component. Not all patients have the characteristics to be subjected to this treatment for this reason surgery still plays a role in this pathology with variable penile straightening techniques with or without the use of penile prostheses.

    TREATMENTS

    Clostidrium histolyticum collagenase

    Collagenase of Clostridium histolyticum is an enzyme composed of two different types of collagenase specifically indicated to degrade the fibrous component of the plaque. The treatment involves inserting this enzyme into the plaque by associating the chemical action of the drug with the physical action of specific manoeuvres. The recovery of the curvature and the improvement of the elasticity will depend on the percentage of calcified tissue inside the plaque.
    Collagenase is still today the only non-surgical treatment for Peyronie’s disease curvature with results demonstrated by the long Florentine experience, first in Europe for the treatment of this pathology.
