Milan penis prostheses help restore normal erectile function of the male external reproductive organ. However, not everyone is aware of the efficacy of surgical therapy and, partly because of misinformation or insufficient information, and partly because of reticence, they prefer not to resort to this treatment.
The cases in which it is possible to intervene with a penile prosthesis are many, but all linked by the same basic component: inability to have and/or maintain an erection. Study and research in this area have made considerable progress in the past 50 years. Whereas during the 1970s-1980s semi-rigid prostheses were mostly used, today hydraulic ones are preferred. Structural materials have changed and surgical techniques have improved.
The insertion of a penile prosthesis leaves a natural appearance to the organ, has no serious complications or risks, and scars are virtually unnoticeable (especially after pubic hair has grown back).
Penile prosthesis: what is it
It is a medical device surgically implanted into the corpora cavernosa of the penis, that is, directly inside the erection chamber. It is a technology adopted to solve a great many cases of erectile dysfunction.

The drugs used to treat this condition are usually PDE5 inhibitors, which is administered with intracavernous injections. Those who do not want to undergo it, or those who cannot, can find a viable alternative in the surgical method.

There are different types of prostheses, which can be grouped into two major macro categories:
- malleable penile prostheses
- hydraulic penile prostheses
Malleable penile prostheses
Non-hydraulic prostheses consist of the insertion of two semi-rigid silicone cylinders, covered with a layer of soft silicone, inside the corpora cavernosa of the penis.
This type of prosthesis was the first to be implanted and tested, since the 1950s. It still gives excellent results today, although the penis always remains in a semi-erect position, but is still easily masked.

As with any other surgery, including penile prosthesis surgery, eating in the six hours immediately preceding it is prohibited. Smoking and drinking alcohol are also prohibited. Every aspect of the patient’s medical condition should be discussed in advance with the surgeon, including whether the patient is taking any medications.
Indeed, in some cases, discontinuation of these medications may be required, even several days before surgery. Resumption of medication should then always be discussed with the physician performing the surgery.
Penis prosthesis Milan: how the operation is performed
Hydraulic penile prosthesis surgery is performed under general, or dorsal, anesthesia. A catheter is inserted to the patient, which is normally removed the day after the operation. Once the patient is anesthetized, the surgeon makes a very small cut at the top of the pubis, at the penis junction, or between the penis and scrotum. This is a really subtle incision, but deep enough to uncover the corpora cavernosa of the penis.
And it is within the corpora cavernosa that the doctor goes to insert prostheses, of a length appropriate for the subject. After the prostheses are installed the reservoir, behind the abdominal wall, and the pump in the scrotum between the testicles, to better hide it.
Once all the elements are placed, they are connected together and the surgeon sutures the incision. Wounds are cleaned and a compression bandage is applied. Many surgeons prefer to leave the penile implant inflated, for at least a day, while others leave the drainage removed after a day has passed.
After surgery: hospitalization, recovery and normal activity
The patient is usually discharged the day after surgery. The bandage is removed and the implant deflated by the surgeon. For a few weeks thereafter, irritation, pain, and swelling may persist. In the two to three days immediately following the operation, there may also be some leakage due to the incision. Problems that solve themselves.
Normal activities, as well as sexual intercourse, are usually possible after 4-8 weeks. During the recovery period, it is recommended not to lift weights greater than 5 kilograms, not to exercise, not to ride a bicycle, and to avoid thermal baths and saunas.
However, recovery times vary from subject to subject and from case to case.
