Curved penis Milan

ANDROLOGY

Curved penis Milan

The male sexual organs, like any other part of the body, are characterized by different shapes, sizes, weights and measures. A rather common peculiarity in men is the curvature of the penis, or recurvatum penile. This condition occurs when the growth of the corpora cavernosa, contained in the male organ, develops asymmetrically, thus creating a longer and a shorter side. Curvature of the penis is actually present from birth, but usually does not appear before puberty. The curved penis is a congenital condition if it occurs from birth. Instead, it is an acquired characteristic, when it manifests itself in the course of life for other causes. The triggering factors are many and everyone can be subjected to a visit and diagnosis by a surgeon who can intervene in the best way to deal with the pathology of the Milan curved penis.

Curved Penis Milan: the characteristics

The Milan curved penis is a fairly common condition. About 1% of the male population suffers from it and it seems that the incidence is always increasing. The penile curvature is present from birth, as a congenital peculiarity and usually develops in puberty. However, it can also manifest itself due to other factors and in these cases we speak of causes acquired.

Whatever its nature, it remains a somewhat embarrassing condition aesthetically and also entails further difficulties during the sexual act. In fact, penetration could be quite difficult and sometimes painful for the partner. It can give rise to episodes of ejaculation premature, erectile dysfunction and even shortening of the penis. To all this we must then add the psychological weight associated with the shame and discomfort that follows.

Statistics and research have shown that this particular morphological anomaly affects about 8% of the world’s male population. In Italy there are ten million men who suffer from it. Most of the subjects who experience cases of curved penis are aged between 40 and 60 years. Diabetics and hypertensives are more exposed.

Curved Penis Milano: how it manifests itself

When the cavernous bodies inside the penis develop asymmetrically, they can lead to a longer and a shorter side, resulting in a curved penis. During the initial phase, the curvature may be slight and appear gradually. Painful or poor erection phenomena are often linked to this condition. The next phase, on the other hand, is characterized by a strong inflammation that affects and circumscribes a more or less extensive area, called “fibrotic plaque”. The fibrotic plaque area, which forms on the tunica albuginea, is often palpable along the shaft of the penis.

The scar reaction determines an effect of retraction and deformation, of various degrees. The degree of inclination varies from subject to subject and from case to case. When the curvature is quite evident, that is to say when it exceeds 30°, in addition to embarrassment, a whole series of other problems must also be faced. In these cases, in fact, sexual intercourse can be difficult, or even painful and, in the worst case, even impossible to sustain.

The curvature can tend to the right or left (lateral), or it can be directed upwards (dorsal), or even downwards (ventral).

Congenital causes

When the phenomenon of the curved penis manifests itself from birth, in a completely natural way, then we speak of congenital causes. These derive from a disharmonious development of the various structures that form the penis: phimosis, paraphimosis, torsion, asymmetry and development of one of the cavernous bodies, etc… Phimosis and paraphimosis can however be acquired from recurrent inflammation (balanoposthitis).

It is above all during puberty that this deformation manifests itself, because it is the period of greatest and most sudden development of the male organ.

Acquired causes

In addition to congenital factors, the curved penis can manifest itself due to many other factors. Among these, the most common and best known is undoubtedly Peyronie’s disease, also called induratio penis plastic. It is a pathology that develops very frequently and consists of the formation of fibrotic plaques on the tunica albuginea, which covers the corpora cavernosa of the penis.

The thickening limits the elasticity of the member which, consequently, has difficulty during the erection and sometimes also during the state of flaccidity. With Peyronie’s disease, the penis tends to curve in the direction in which the fibrotic plaque has formed.

But this is not the only acquired cause that can lead to penile recurvatum. For example, even post-traumatic situations can generate the pathology. Sudden, abrupt and painful bending of the penis during the sexual act are among the other causes attributable to the phenomenon. In still other cases, however, it is ruinous sports injuries that have affected the pelvis, which make the onset of the morphological anomaly possible.

Furthermore, all the consequences of post priapism or of different diseases that compromise blood circulation in the penis, such as penile mellitus, are also to be remembered. Autoimmune disorders and abnormalities related to inherited fibrous tissue (collagen) can lead to developing a curved penis.

TREATMENS

Curved Penis Milan: treatments and surgeries

Usually this pathology is initially addressed with a pharmacological treatment with anti-inflammatories to be taken orally. This serves to reduce the scar reaction and the pain-related component.

Subsequently, when the acute inflammation has ended, the pain has decreased and the curvature has stopped or stabilized, possible treatments with local infiltrations of the scar area are started. In other cases, however, low-intensity shock waves are used to stimulate the blood vessels locally, regenerating the tissue. The waves also lead to the elimination of pain by acting directly on the plaque, consequently improving its deformation and curvature.

These are non-invasive therapies that can be good in some specific cases. However, when these treatments do not lead to concrete and satisfactory results, the only other possible solution is to resort to penile surgery. This actually happens even when the bending of the shaft of the penis is equal to or greater than 45°.

In any case, the best thing is to contact a urologist, or a andrologist who can specifically analyze the case. These will evaluate, on a case-by-case basis, whether it is necessary to resort to a targeted surgery to resolve any pathology.

TREATMENS

Collagen: non-surgical treatment

The pathology of the curved penis can also be treated without resorting to surgery. Through collagen treatment it is in fact possible to achieve remarkable results, in a relatively short time.

Collagenase of Clostridium histolyticum is an enzyme formed by two different types of collagen. These components are able to progressively destroy the fibrous component of the plaque. The technique involves the insertion of this enzyme into the fibrotic plaque. Its action is combined with the physical action of some specific maneuvers, thus allowing a rapid recovery of the normal penile curvature. The recovery of the curvature and the improvement of the elasticity depend on the percentage of calcified tissue which is found inside the plate itself.

Treatment with collagenesis is to date the only non-surgical intervention to cure the pathology of the curved penis and the Peyronie’s disease.

    Dr. Andrea Cocci

    urologist and andrologist

The decision to dedicate my professional life to urology and in particular to andrological and reconstructive surgery is the result of a deep passion for anatomy , the art of surgery and in general the diagnostic-therapeutic process which leads to the recovery of the patient. Oncological pathology, infertility, erectile dysfunction, penile diseases or simply prostate disorders irreparably afflict not only the individual but also the couple dimension.
